I hereby certify that I am the rightful owner/keeper/caretaker/custodian of the animal(s) listed below. I herby acknowledge and certify that I am licensed to own below animal(s). I certify that the animal(s) listed below were obtained legally and not taken from the wild. I hereby surrender any and all property rights to the animal(s) listed below. I certify that no other person has a right of property to the animal. I understand that by signing this I am transferring ownership and property rights of the below animal(s) to Walking Wild Rescue Inc. I understand that hitting send is serving as my digital signature. I understand that once this document is signed/sent, the animal(s) listed below will not be available to be returned. I further certify that I have read and understand the terms of this intake form. 

 **Please note - link is shared after conversation of surrender and intake is agreed upon. If more than two weeks passes between receiving this link and completing the form, unless Molly has been made aware of the delay in writing, the form is no longer valid if filled out. Due to the need, we can not hold space or guarantee we still have room if the form is not submitted within the two weeks or agreed upon time frame. Filling out this form after the two weeks or agreed upon time frame is not valid and surrender/intake is not guaranteed.**
